To request a room at the Hope Lodge in Lexington, you must be referred by your Lexington area physician or health care professional. Your health care professional must complete the referral form below and submit it to the Hope Lodge via fax at 859.260.8302. The Hope Lodge staff will coordinate with your health care professional to add your name to our waiting list. It is advisable to initiate the referral process well in advance of the first date of treatment since patients are accepted on a first-come, first-serve basis. Hope Lodge Referral Form The process for medical professionals to refer patients is as follows:
- The medical professional must assess if the patient meets all eligibility requirements.
- If the patient does meet all eligibility requirements, the medical professional must submit a patient referral form.
- All referrals are processed on a first come, first served basis and serve all cancer treatment centers within the given geographic service area, providing equal access of service to their patient base.
- Referral forms must be legible and properly completed/signed by the referring medical professional.
- A referral does not guarantee reservation at the Hope Lodge, as at times, there is a waiting list.
- Priority is given to patients needed lodging for three or more nights to receive a minimum of three treatments.
- Patients and caregivers may be admitted no sooner than one day prior to treatment initiation and must vacate the Hope Lodge no later than 24 hours following completion of treatment.
- For continuance occupancy of a guest room, patient must receive out patient treatment a minimum of 3 days per 7 day period.
- Hope Lodge Staff will require a new referral form any time there is a treatment change. Guests or the medical professional must notify Hope Lodge staff of any change in treatment plan and duration.
- Once the referral form is completed, it must be faxed to (859) 260-8302.
- Once the form is received, Hope Lodge staff will assess if space is available for the patient as well as if they meet eligibility requirements.
- The medical professional will receive a call from Hope Lodge staff confirming the receipt of the form and whether we can accommodate the patient.
- If the patient is approved, they will receive a confirmation letter in the mail that will include directions to the facility and information on our rules and regulations and the registration process.
In rare cases, when space allows and with Division Hope Lodge Director approval, the Hope Lodge Director may use his or her discretion to make exceptions to the eligibility criteria.
